CONTEST RULES

The “Customer Survey” contest is organized by Mode Choc, whose head office is located in Alma, Quebec.

The “Mode Choc Survey” contest begins on August 1, 2026, and will continue until modified or terminated by Mode Choc.
Each monthly entry period begins on the first day of the month at 12:01 a.m. (ET) and ends on the last day of the same month at 11:59 p.m. (ET).
To be eligible for the monthly draw, participants must complete the survey during the applicable monthly entry period.

The contest is open to residents of Quebec and New Brunswick who are 18 years of age or older.
Excluded: employees of Mode Choc and any individuals residing at the same address as them.
Only one (1) entry per person per monthly entry period is permitted.
The entry must be completed in the participant’s own name.

Complete the Mode Choc survey received by email or accessible through the QR code printed on a sales receipt during the applicable monthly entry period.
Complete all required fields on the entry form, including the invoice number.
Each completed survey automatically provides one (1) entry into the draw for the current month.
No purchase is necessary to enter or win.
Only one eligible entry per person per month will be accepted.

One (1) prize will be awarded each month from among all eligible entries received during the applicable monthly entry period.
The winner will receive one (1) Mode Choc gift card valued at $50.
Approximate monthly prize value: $50.

Prizes are non-transferable, non-refundable, and cannot be exchanged for cash or any other goods or services.

The draw will be held on the 7th day of each month at Mode Choc’s head office in Alma, Quebec.
If the 7th falls on a weekend or statutory holiday, the draw will be conducted on the nearest business Monday.
The draw will include all eligible entries received during the previous month.
Winners will be contacted by email and/or telephone within 10 business days following the draw.
The winner’s name may be announced on Mode Choc’s Facebook and Instagram accounts after the winner has been contacted.

To be declared a winner, each selected participant must correctly answer a mathematical skill-testing question included on the entry form.

The $50 gift card will be awarded to the winner at the Mode Choc store of their choice or mailed to their residential address if in-store pickup is not possible.

Before receiving the prize, each winner must provide valid government-issued photo identification.

The personal information collected (name, address, email address, telephone number) will be used solely for contest administration, contacting winners and prize distribution, in accordance with applicable laws and Mode Choc’s Privacy Policy.
By entering the contest, participants consent to the use of their information for these purposes.
Information will be retained only for the period necessary to administer the contest and will not be sold or shared.

Mode Choc reserves the right to verify participant eligibility and to disqualify any incomplete, fraudulent or non-compliant entry.
Mode Choc, its employees, agents and service providers cannot be held responsible for any technical issues, printing errors, loss of data or any other event beyond their control that may affect participation or contest administration.
Mode Choc reserves the right, at its sole discretion, to modify, suspend or terminate the contest if an event beyond its control prevents its proper administration.

Mode Choc reserves the right to terminate the contest or modify its mechanics, prizes or draw dates at any time, without prior notice, subject to applicable legal requirements.
